Microneedling: the honest balance
Potential advantages

- Nobody has published good evidence on how long results last beyond about six months, so maintenance is a realistic expectation rather than an upsell
Almost every description of microneedling reaches for the same reassuring phrase: it stimulates your skin’s natural healing. True, but it skips the part that matters. Microneedling works by injuring you, deliberately, thousands of times in a few minutes, because a controlled injury is the only reliable way to make adult skin build new collagen. Understanding that one idea explains everything else about the treatment: why it needs a course rather than a single session, why the results take months to mature, and why depth is the number that decides both how well it works and how safe it is.
What microneedling actually is
Microneedling, known clinically as percutaneous collagen induction or collagen induction therapy, uses very fine needles to make controlled, closely spaced punctures through the outer layer of skin and into the dermis beneath. Each puncture is a micro-channel. Thousands of them together are read by the body as an injury requiring repair.
The crucial detail is what is not damaged. Unlike ablative laser resurfacing or a deep chemical peel, microneedling does not remove the epidermis, and it leaves the basement membrane beneath it essentially intact. That is not a side note, it is the entire reason the technique exists. The South African plastic surgeon Des Fernandes began treating patients this way in 1997 and set it out in the surgical literature explicitly as an alternative to laser resurfacing, one that could be repeated because the skin’s surface was preserved. Preserving that surface is also the mechanistic basis for its comparatively low risk of pigmentation problems, which is what makes it usable across a much wider range of skin tones than more aggressive resurfacing.
Modern practice has largely moved from the drum-shaped rollers of the 2000s to motorised pens with adjustable depth and single-use sterile cartridges. That change matters more than it sounds, for reasons the safety data make plain further down.
The principle: injury as a signal
Puncturing the dermis sets off the same three-phase healing sequence as any other wound, just at a scale small enough to be invisible. First comes inflammation, with the release of the growth factors that summon repair cells. Then the proliferative phase, in which fibroblasts migrate in and lay down type III collagen, elastin and the ground substance that holds the tissue together, alongside new blood vessels. Finally comes remodelling, over months, in which that provisional type III collagen is progressively converted to the stronger type I that gives skin its structure.
This is why microneedling cannot be rushed. The visible change is the end of a months-long remodelling process, not the result of the appointment itself.

Depth is the variable that decides what any of this achieves. Needling at 1.5 mm produces new collagen measurable at depths of roughly 5 to 600 microns. Go shallower and you are treating the surface only. One widely quoted figure claims an up-to-400 per cent increase in collagen and elastin six months after four sessions, and it does appear in the literature, but it is a maximum reported in small series rather than a typical outcome. Independent work is more sober: one study of eleven patients found a clear increase in elastic fibres in about two thirds of them and no statistically meaningful increase in skin thickness at all. Both findings are real. Only one of them tends to make it into advertising.
It is also worth knowing that needle length is not the same as needle depth. Skin tents under pressure, and the speed and angle of the device change how far the needles actually reach, which is why two clinics quoting the same setting can deliver different treatments.
Where it sits among the alternatives
Microneedling is routinely confused with two other things, one more expensive and one far cheaper.

The honest position on home rollers is that they are reasonable as superficial exfoliation and to help products penetrate. They are not collagen induction therapy, and buying one is not a cheaper version of a course.
What the evidence actually supports
The strongest indication by a distance is atrophic acne scarring. A systematic review of 33 studies found improvement in every single one, and a later network analysis of 24 randomised trials covering more than 1,500 patients ranked microneedling combined with chemical peels as the best-performing strategy of those compared. That said, when the randomised trials are pooled properly the objective effect is statistically significant but numerically modest, so expect meaningful improvement rather than erasure.
For lines and wrinkles the best data come from the trials that supported device clearance. In the SkinPen study, four monthly treatments improved all twelve assessed facial areas under blinded assessment, with cheek fine lines improving in 87.5 per cent of patients and clinicians rating 81.3 per cent improved overall at three months. Two caveats belong alongside that: only 32 people completed it, and three of its authors worked for the manufacturer. The separate Exceed trial is more useful for setting expectations, because it recorded where the treatment did not work. Around 85 to 90 per cent of patients improved in the glabella, around the eyes and in the cheek folds, with nobody worsening, but there was no clinically meaningful improvement in horizontal forehead lines, nasolabial folds or upper lip lines.
Beyond the face, one study of 25 patients treated for stretch marks across a wide range of skin tones found that all of them achieved at least 50 per cent improvement, after an average of fewer than two treatments. Microneedling is also used as an adjunct to topical minoxidil in hair loss, where the combination clearly outperforms minoxidil alone, though that is an off-label use of a device with no authorisation for the purpose.

What a course involves, and what it costs
Guidance converges on three to five treatments spaced two to four weeks apart, with the British College of Aesthetic Medicine suggesting three or four at roughly six-week intervals and up to six for scars and stretch marks. Sessions run 20 to 30 minutes.
Downtime is real and short. Expect two to seven days. In one regulatory trial every participant was red immediately afterwards, though 81 per cent had cleared by day eight, and every participant peeled between days three and eight. Improvement was measurable a month after the final session and held for at least 90 days, which was as long as anyone followed it. Beyond six to twelve months there is very little good durability data, so any claim of multi-year results is running ahead of the evidence.
In the UK a single professional facial session sits broadly between £145 and £300, with courses discounting the per-session price by roughly 10 to 25 per cent.
Risks, and who should not have it
Most reactions are transient redness, swelling and discomfort that settle within about a week, and the largest safety review of 85 studies concluded microneedling is a relatively safe therapy. The specific risks worth knowing are narrower than the general disclaimers suggest. Pooled data show post-inflammatory pigmentation in about 6.8 per cent of roller-treated patients and tram-track scarring, a line of small raised marks following the device’s path, in about 10.5 per cent, which is a strong practical argument for pen devices over rollers. The most striking serious complication in the literature is a delayed granulomatous reaction repeatedly linked to vitamin C serum applied during or straight after treatment, a reminder that anything put on freshly needled skin is effectively being delivered into it. And in one trial, of the patients prone to cold sores, 62 per cent had an outbreak, so tell your practitioner if that is you and expect antiviral cover.
Microneedling is not suitable if you have a tendency to keloid or hypertrophic scarring, are taking isotretinoin, are immunosuppressed or heal slowly, are pregnant or breastfeeding, are on anticoagulants, or have an active skin infection in the area. Under-18s should be declined.
One regulatory point deserves stating plainly, because clinics rarely volunteer it. There is currently no national licensing scheme for microneedling in England: the Health and Care Act 2022 created the power to introduce one, and the Government has proposed a tiered system, but it is not yet law. What does bite is the national occupational standard, which limits beauty and aesthetic therapists to 0.5 to 1.0 mm on the face and 1.5 mm on the body, and treats anything deeper as medical work outside their scope and likely outside their insurance. Different training frameworks set slightly different limits, which is itself a reason to ask directly.

Is microneedling FDA approved?
No microneedling device is FDA approved. SkinPen was authorised through the FDA's De Novo route in 2018 and others have been cleared through the 510(k) route since. Those are different, lower evidential bars than approval, and in any case US clearance has no legal standing in the UK. Be wary of any clinic that uses the word approved.
How many sessions of microneedling will I need?
Most guidance lands in the same place. The American Academy of Dermatology suggests three to five treatments every two to four weeks, and the British College of Aesthetic Medicine commonly three or four about six weeks apart, rising to six for scars and stretch marks. The trials behind the cleared devices used four treatments thirty days apart.
Is microneedling the same as Morpheus8?
No. Morpheus8 is radiofrequency microneedling, which adds heat delivered through the needles. It is a different treatment with a different evidence base and different regulatory attention. In one meta-analysis of randomised trials for acne scarring, plain microneedling reached statistical significance and fractional radiofrequency microneedling did not, so the two should not be treated as interchangeable.
Are at-home dermarollers worth it?
Not as a substitute. Home rollers are typically shorter than 0.15 mm, which does not reach the depths at which the published collagen research was generated, and no microneedling medical device is authorised for over-the-counter sale. There are also documented cases of infection and viral spread from home devices. Treat them as superficial exfoliation tools, not collagen induction therapy.
