You can buy a dermaroller for about thirteen pounds. The device a clinic will use on you costs thousands, is classed in the United States as a prescription instrument, and has a category in the federal device code that did not exist before 2018. Both are descendants of the same idea, and the distance between them is not marketing. It is thirty years of a single problem being solved: how to injure skin precisely enough to make it rebuild, without damaging the surface you are trying to improve.
The idea before it had a name
Microneedling’s ancestors were scar treatments, and they arrived from two directions at almost the same moment.
In 1995 Orentreich and Orentreich described subcision, passing a needle under a depressed scar to cut the fibrous tethers holding it down and provoke new connective tissue to form in the space. Two years later Camirand and Doucet reported something cruder and, in hindsight, more prophetic: they ran a tattoo pistol over scars with no pigment in it. The needles alone, repeated thousands of times, remodelled the tissue. That established the principle the whole field now rests on, which is that the puncture itself is the treatment.
In the same year, 1997, the plastic surgeon Des Fernandes began treating patients in Cape Town with a technique he called percutaneous collagen induction. He was not trying to sand skin down, which is what resurfacing did. He was trying to trigger repair while leaving the surface intact.
Fernandes and the case for leaving the epidermis alone
Fernandes set the technique out properly in a 2005 paper and then in a 2006 paper whose title was the entire argument: percutaneous collagen induction as an alternative to laser resurfacing. His pitch was modest in a way that has not survived into modern advertising. A single treatment, he accepted, might not match laser resurfacing. But the epidermis remained virtually normal afterwards, and because it did, the treatment could simply be repeated.
That trade, less per session in exchange for being able to do it again safely, is the reason microneedling exists as a category. It also explains the course structure clinics still use today.
The tool he built for it was a drum. The Medical Roll-CIT was a hand-rolled cylinder studded with needles, reusable, and in its early-2000s form fitted with needles up to 3.0 mm long. For roughly a decade, that was microneedling.

The 2008 study everyone still quotes
The paper that turned a technique into a movement appeared in Plastic and Reconstructive Surgery in 2008. Aust, Fernandes and colleagues reviewed 480 patients treated in South Africa and Germany for fine wrinkles, lax skin, scarring and stretch marks. Most had a single treatment. The German patients rated their own improvement at 60 to 80 per cent. Biopsies from twenty of them showed a considerable increase in collagen and elastin six months on, and at one year the deeper layer of the epidermis had thickened by around 40 per cent, with the skin’s natural ridged architecture intact rather than flattened.
It is a genuinely important paper, and it should be read with its own small print. It was retrospective rather than controlled. The headline improvement figure was patients scoring themselves. And every patient had been using topical vitamin A and vitamin C for at least four weeks beforehand, which means the study never tested needling on its own.
The same team published a second 2008 paper that has had an even longer afterlife in marketing. It was a controlled study in 56 rats, and it found that the epidermis, the outermost layer and the basal membrane were undamaged, with melanocyte numbers neither raised nor lowered. That is the origin of the claim, repeated on clinic pages everywhere, that microneedling carries no risk of pigmentation change. It is a reasonable mechanistic finding. It is also a rat study, and the species rarely makes it into the sentence that quotes it.
Why clinics moved from rollers to pens
Through the 2010s, motorised pens with vertically oscillating needles displaced the roller in most clinical practice. Two things changed, and both matter more than the marketing did.
The first was dosing. A standard roller carries 192 needles and lays down roughly 250 punctures per square centimetre after about fifteen passes, which depends entirely on how the practitioner rolls. Skin tents under pressure, so how far the needles actually reach varies with force, angle and speed. A pen sets depth as a number. The SkinPen, for instance, offers eleven settings in 0.25 mm steps.
The second was sterility. Pens use disposable, individually packaged cartridges, and the FDA is explicit that these are single-use and must not be reused between patients. A drum roller was cleaned and used again.
The safety record follows the engineering. Pooled data across 51 studies record less immediate redness from rollers than from pens, but rollers account for the complications that do not fade: post-inflammatory pigmentation in about 6.8 per cent of patients, and tram-track scarring in about 10.5 per cent. That second one, a row of small regularly spaced raised marks tracing the device’s path, was first described in 2012 in a 25-year-old woman treated for acne scars. Redness settles in a week. Tram-track scarring is a scar.

Regulators arrive, in one country
For twenty years microneedling had no regulatory identity at all. That changed in March 2018, when the FDA granted the first authorisation any microneedling device had ever received, a De Novo for the SkinPen Precision System. Because nothing comparable existed, the decision created a new classification for microneedling devices used aesthetically, and made them prescription instruments. The original cleared claim was narrow: improving the appearance of facial acne scars in adults aged 22 and over.
Later that year the Exceed device became the first cleared for facial wrinkles, and in 2021 SkinPen added neck wrinkles. Alongside the authorisations came limits worth knowing: the SkinPen’s safety and effectiveness were never evaluated above 1.5 mm, and no more than 0.5 mm is permitted over the bony rim of the eye socket.
None of this is UK law. Aesthetic devices without a medical purpose have historically fallen outside the UK medical devices regime, although the Government has said it intends to bring them into scope. What governs your appointment in Leeds today is not a device authorisation but the national occupational standard, which caps therapists at 0.5 to 1.0 mm on the face and 1.5 mm on the body and treats anything deeper as medical work.
What the history is actually telling you
Read as a sequence, the last thirty years are one long argument that control is the active ingredient. The idea was proved with a tattoo pistol. It was popularised with a hand-rolled drum and a study that flattered it. It was refined by making depth a setting and the needles disposable. And it was finally recognised by a regulator that also drew a line around what the evidence supported and what it did not.
The branch that added heat, radiofrequency microneedling, is a different treatment with its own evidence base rather than a newer version of this one.
So the useful questions in a consultation are the ones this history keeps returning to. What device, at what depth, by someone qualified to work at that depth.
